Pesanan MO (Garduan Mesir) kepada Junior-junior

Graduan Mesir

“Abang tahu tak, sekarang ramai orang duk komplen graduan Mesir bermasalah” ujar isteri kesayangan suatu hari.



“Kenapa?” Tanya saya kembali.



“Ada seorang tu, bla bla bla...” bebel isteri saya seperti selalunya.



Saya tidak pasti apa perasaan saya apabila perkara itu diutarakan kepada saya. Sedih? Bimbang? Marah? Pasrah? Secara jujur saya tidak berhadapan secara langsung, memerhati atau menyelia prestasi HO-HO yang dikatakan bermasalah ini memandangkan jabatan saya sekarang tidak ramai bilangan HOnya dan tiada pula dari Mesir. Namun menurut kata-kata dan bualan sahabat-sahabat memang menyebut bahawa graduan Mesir bermasalah.



Jadi, apa puncanya?



1. Prestasi : Takut untuk clerk kes, takut untuk buat procedure, takut untuk present kes.

2. Chipsmore : Sekejap ada kat wad, sekejap ada kat OT, sekejap ada kat pantry, sekejap ada kat bilik oncall.

3. Praktikal: Tidak reti pasang branula, tidak reti baca CVP, tidak ingat nama ubat.

4. Attitude: Melakukan kesilapan yang sama berulang-ulang. Tidak bertanggungjawab. Lari/mengelak dari melakukan tugas.



Bercakap melalui pengalaman, saya akui sememangnya terdapat beberapa disadvantage yang saya rasakan jika dibandingkan diri saya sebagai graduan Mesir dengan rakan-rakan daripada local university atau negara lain tapi kebanyakannya dapat saya atasi apabila dapat menamatkan posting pertama saya di Medical. Bila mula posting kedua di O&G, saya dapat merasakan bahawa saya tidak berada jauh di belakang kawan-kawan sehouseman yang lain.



Semasa tagging first posting itulah hampir semua benda saya baru nak belajar. Dan sepanjang 4 bulan di posting pertama itu bermacam-macam sindiran yang saya terima hasil dari ketidakretian saya itu. Masih ingat seorang MO bertanya kepada saya pada hari ketiga tagging, “kau dah berapa hari dah tagging? Dah clerk kes belum? Nak tunggu berapa lama baru nak clerk kes?”



Saya pernah dimarahi kerana tidak prescribe ubat-ubat malah tidak baca pun clerking MO yang melihat pesakit MI di ED yang kemudiaannya dimasukkan ke CCU.



Saya pernah disuruh oleh MO untuk menulis sebuah buku kecil dan menyenaraikan semua common drugs yang digunakan di wad medical kerana asyik terlupa dengan ubat dan dose yang sepatutnya diberikan kepada pesakit.



Saya pernah diingatkan oleh MO bahawa pandangan MO terhadap saya akan sedikit sebanyak mempengaruhi keputusan pakar untuk mengextend atau tidak posting saya di medical itu.

Melihat kepada pengalaman yang saya lalui sendiri, apa nasihat yang boleh saya berikan kepada adik-adik yang baru bergelar houseman ini?



Pada saya, kekurangan yang kalian ada itu sebenarnya tidaklah begitu teruk kesannya sekiranya kalian punya sikap dan attitude yang betul sebagai seorang houseman. Houseman posting adalah tempoh untuk kalian belajar dan bersedia menjadi seorang doktor yang selamat (safe doctor). Justeru bersedialah untuk itu. Sikap yang betul termasuklah:


1. Belajar dari kesilapan.

2. Menunjukkan minat melaksanakan kerja.

3. Bersungguh – sungguh bekerja.

4. Berkomunikasi dengan baik.

5. Berbaik-baik dengan rakan sehouseman, MO dan specialist

6. Tidak menyusahkan orang lain (kawan-kawan HO)

7. Tidak mengelat/mengelak dalam melakukan kerja.

8. Terbuka kepada kritikan.

9. Menggilap skill dari hari ke hari.

10. Dan yang paling penting, walau apa yang kalian lakukan, don’t jeopardise your patient’s life.



So, adik-adik. Saya percaya kalian takdelah teruk sangat. Cuma tempoh untuk kalian adapt dengan alam pekerjaan mungkin berbeza. Senior-senior kalian pun tak adalah perform sangat semasa zaman HO dahulu, cuma at the end, Alhamdulillah dan InsyaAllah kami berjaya menjadi MO yang baik dan mudah-mudahan at least a safe doctor. Apa yang penting berbalik kepada sikap korang la ya. So, selepas memberikan yang terbaik semasa zaman undergrad, berusahalah juga untuk memberikan yang terbaik di alam pekerjaan ini. Tak ramai yang dapat menolong kalian selain diri kalian sendiri. Selalu senior-senior sebut, “bila dah dapat survive kat Mesir, campak kat mana-mana pun insyaALlah boleh survive.” So buktikanlah yang itu insyaAllah.



Selain memikirkan kekurangan yang kalian ada, manfaatkanlah juga kelebihan yang kalian ada dari segi teori , kebolehan bercakap bahasa arab (akan dicari bila ada patient Arab admit/nak bersalin etc) dan kemahiran-kemahiran lain yang mungkin kalian dapat sepanjang berada kat Mesir haritu (berpersatuan/IT/bersukan/muzik/penyiaran dll) semuanya adalah nilai tambah yang kalian ada dan perlu pandai mencari channel untuk serlahkan dan tonjolkan dengan sebaiknya. So, don’t feel stressed, think positively and be happy insyaAllah.



Uish, teruknya tulisan saya kali ni. Lama tidak menulis, teramat kekok rasanya, apapun all the best untuk kalian dan moga dipermudahkan segala urusan.



Ps: Kalau refer case or notify case tu kene marah2 dengan Dr Kadir tu jangan simpan dalam hati la ya. Nothing personal :P. Tapi kalau kene marah dengan bini saya tu memang personal. Dia memang garang!

Operation Theatre (again? :o) @ ZUH

Eh, I was in hospital ;)




8/8 1st day: know about department, staff

 9/8 2nd day: Library

10/8 3rd day: join a talk on mental healthiness at UPM ( talk delivered by three doctors from other hospital )

11/8 4th day:  talk on “Missing Bone” by orthopaedics specialist, prepared myself to enter operation theatre, but...huhu..i don’t know how to scrub my hand..i didn’t know how to open that pipe...ok..Tell u..i was awkward. This is first time..ok,, then i remembered the Korean medical drama, New Heart..the surgeon will kick the bottom of iron sink. Not kick. However, gentle than kicking. Huhu..so i did..wow.surprised..the water came out splashingly..alhamdulillah. Then i followed the instruction on how to scrub the hand. (htere was an article on the wall..but to read..it consumed time. Awkward.. Really..then i didn’t know how to take out the fluid soap from the bottle..uh..really..killing me..i thought it was not my time. also the OT shirt that i wore got wet at the end of the shirt arm..only few centimetres then to enter. I just observed the operation about few seconds through small glass window (the square one)..

12/8 5thday: hurray, today I entered OT;) Alhamdulillah..i reported myself to the head nurse and the surgeon. The surgeon will do total hip replacement surgery. ( I was not planning to go there, but just went to changing room, then asked the doctor (HO@MO) that also came to change clothes for surgery to join them;). 2nd OT, i wanted too, but the all  robes (robes-like that prevent us from radiation, i didn’t know what it was called) wore by doctors and surgeons, so no more robes left for me ..the surgeon registered to do surgery was only Dr. H, but his friends came. I mean Dr. H’s friends were also surgeon so they did surgery together, about two friends kot..diorang saje..lepaking tapi also helping la sambil berbual...so, the HO only did observing like me too..hehe ( I thought they were serious like in dramas, but no..Here they also had chit-chat too..hehe..no stress..siap gurau2 lagi dalam OT.

15/8 6th day: studied at home

16/8 7th day: Library. Take some contacts. I just met some medical students from..em this time, I forgot. Myb from Cyberjaya Medical College kot.

17/8 8th day: holiday (Nuzul Quran )

18/8 9th day: OT. Urology 9am till 11.20am (urinary stone case) Ophthalmology  OT 12-2.00pm (cryptophthalmus case)

19/8 10th day: absent. Overslept

22/8 11th day: today was the first day at Cardiology department. Library

23/8 12th day: Library (studied on ECG)

24/8 13th day: Cardio Clinic..I was “attacked” by many questions..Felt dumb. uh. What is....How to diagnose...??bla..bla.bla....Have u learned about heart sound? Come here. Try to listen. Me: no doctor, i not learned yet.. Doc: that’s ok..i’ll put the diaphragm (stethoscope) on his chest..Me:ok....Dr: u heard the sshhruuu...dup...shruuu dup?? It is aortic valve regurgitation...Me: oh, i see ;)

Tip for female doctor: please do not twist your scarf/shawl tightly. That complicated fashion of hijab... Coz when you want to put ur stethoscope dekat telinga, susah sangat..i took few seconds to adjust my shawl here..huhu.(nasib baik doktor India ni sabar, baik, lembut dan gentleman ;) 

25/8 14th day: Procedure room. Observed echocardiogram

26/8 15th day: Absent. Overslept

29/8-------------4/9 at village.......celebrated Raya Eidulfitri.....................................

5/9 16th day: Rehabilitation department. (Include Physiotherapy)

6/9 17th day: Absent. Overslept & Feeling not well

7/9 18th day: Observed some patients. Took history

8/9 19th day: Library. Treatment room. As usual, took history of back pain patients, stroke.

9/9 20th day: Library. same. but always got something new. Every patient was different. Autism, slow learner, etc..etc ;)

12/9 21th day: Rehab. I Observing some patients. “Escaped” to medical ward (two patients, one was pulmonary TB + myeliodiosis, one more emm forgot already)..hehe;)

13/9 22th day: Library. Got Cerebral Palsy patient (I jotted down my experience handling this case previously)

14/9 23th day: Library. Read some about stroke rehabilitation. The book was seriously boring. No pictures, no tables, no graphics..so, I read another book and slept. It was common here to see nurses slept, MO slept, I slept..HO?? emm, not sure..so i just imitated them..

15/9 24th day: Hospital Auditorium. ATalk on Cardiopulmonary Resuscitation. The anaesthetist Mrs. R went on a meeting in Penang. They discussed about some latest issues regarding CPR (the updated one).The talk was marvellous!! Like..like..like

16/9 25th day: it was my last elective posting day..but i sacrificed it to Malaysian..You must thanked me. Ade-ade je. Padahal memang cuti pun..1 Malaysia. I like the song by Ernie and the yellow hair man..ape la..nyanyi lagu Malaysia, butt, what happen to their hair??? Kalau I, I malu nak nyanyi lagu Malaysia, tapi rambut oren..tak sah mandi wajib..for sure, lainla kalau pakai inai kan, tapi inai mestilah remeh sedikit..but, may i ask u, what is the benefit that you can get by colouring / dye..eh dying? Your hair?? RM 15o kot kalau colour..pastu, nak mandi wajib pun susah..kena potong rambut dulu ke?? Nvmd~

Finish writing..tomorrow will go to Ampang Puteri to see dentist..
I want to throw out my adik bongsu there..ehh, no, gigi bongsu...i asked my mom, kenapa Allah jadikan gigi bongsu kalau tiada fungsi? The dentist that i met before, i mean today( yeah, penat ni baru balik) at Al-iSLAM Specialist Hospital Kuala Lumpur said that gigi bongsu tiada fungsi..betulkah? Ia mungkin akan mengganggu gigi-gigi kakak dan abang yang lain..em..wondering~

Ignore my overslept. It was good enough to do elective posting almost a month.ye tak?? Hehe..On 1Malaysia day, I felt relieved. True. I really meant it..;)

A Sunny Day in Paed Gymnasium Room (CP)

after finished breakfast with my dad, mom and Ina at hospital cafe ('lepak'ing), 9.10 am, I rushed to the w/c to clean my oral cavities..as usual, I looked into the mirror...put on white coat neatly and walked without thinking anything. (usually i suka gak planning/dreaming ke masa berjalan..quite fun;)..

then, entered in clinic and observed some patients..

if no patients in orthopedic therapy, or if i get boring sitting in there, i escaped..hehe

to cafe?? no..
to garden?? no..
to anywhere?? yes..hehe..

anywhere means anywhere in the departments..so, when i saw patients' sandals in front of the door, i quickly rushed in..

now, i in paediatric gymnasium (haha..jgn main2..bdk2 pun ade gym)..Chinese baby in bicycle-like tool..didn't knew what was that, but as I know, it is therapy for the child legs to use their legs..means a child has grown up already about two yrs for example, but he or she didn't use his/her leg. They tend to sit or just stay in supination, so use this kind of therapy to stimulate the legs to stand and walk..

i forgot the signs & symptoms of CP
i not prepared myself yet to encounter paediatric patients. i not learning paed except in embryology in 1st and 2nd year..

so, i asked the therapist about CP
tiba-tiba...'haah, inilah dia..kenapa awak tak tahu pasal CP, awak nak saya spoonfeed awak ke? (sambil menunding jari ke arah muka saya) awak ingat katsini nak mengajar semua ke? awak tak tengok ke ape dia simptom
saya: saya baru masuk, tak belajar paed lagi.. (nada perlahan, tapi still boleh dengar)
dia: awak medical student kan, i expect more from u, ..bla..bla...kalau awak tak tahu, pegi la baca buku..nak tengok kes, tapi tak tahu


korang expect apa? saya menangis..hewhew..off course not..

i felt like i was on the sky..like 'eh, betulke ape yang aku dengar ni'...em, siap menyampuk lagi masa dia membebel. sekejap je. tak lama dia bebel (macam sama je perangai kita berdua..membebel..hehe..)

then, i got myself back. i saw people around me. other therapists, therapist student, child's mom and his grandmom, a __ha? gentleman?? is he handsome or...oo..ok..like he was in secondary school..nasib baik, kalau tak, malu dowh muka aku ni if he just around my age....now, i could feel how HO feel..

kadang2 i feel nothing..lantakla kau nak marah pun, aku nak belajar

so, i touched the other CP patient. Malay. Grasp reflex  was still present and some more other reflexes..si dia yang marah aku tu pun, observed what i was doing..only Grasp reflex i knew..but another reflex, i didn't know what it was called..yeah, frustrated. she asked me about the reflex name...

she went to table then said 'hai, tak tahu ape2 la'

i heard she scolded ke adviced, menyindir mungkin the therapist student..
kadang-kadang, lawak je therapist yang marah aku ni..

kadan-kadang aku suka cikgu yang kat sekolah tu garang sebab sekali ajar dah masuk dalam kepala..

patient da balik, i moved out to library..mencapai buku paed :
sign and symptoms, congenital anomalies, headache in paed
i found essential of paediatric churchill livingstone was helpful

belek2 , jumpa la CP ni..baca sket..
about 4++ pm, mom called. went to lobby..

balik rumah la off course ;)

trivia¬ in embryology textbook 1st or 2nd year , i remembered the figure of CP..but the figure show only wind-swept deformity (it is due to patient not undergo/delayed treatment/therapy), but today, i saw the clenched fits sign only..because wind-swept deformity tu yang memang da teruk da..
so stages yang lain..at first glanced, really, i didn't knew he suffered from CP..i knew it just only by ..guessing..hehe

(boleh cari kat internet clenched fits tu ape and WS deformity tu ape..i try to upload here, tapi macam menunggu hujan, lambat..myb laptop da panas, sebab tak bawa balik cooler pad..ok;)

really..i admit..i'm lacking~ but i/we still learning..
keep moving on..insyaallah;)

A chance





assalamualaikum readers


someone saying~"Junior doctor", " Student Doctor", " Young Doctor" are some of the words our lecturer usually use to introduce us to the patients. Why don't they just use "medical student" or "pelajar perubatan"?

Because these patients are somehow selective. They want to be seen by real doctors with license.
THeir body is just more precious than diamonds and their mouths were sealed by gold. THey won't let medical student examine or take history.

THey think that the present doctors that they are seeing now are born as doctors. THey doesn't undergo training, they have never been medical students before. THey born naturally and gifted with skills to clerk history, to examine, then to treat, and to operate. In other words, medical student will be medical student for ever. Doctors will be doctors for ever. And, we can wait in O&G labour room for babies that will be born as doctors for the future. Once a baby is born, we dont test for Apgar score, but we test their medical knowledge. Ok, pass, u r a doctor. Sorry, fail, u will be trained as medical student and will suffer and face difficult patients...so unfair!!!haha...then if im parents i'll pray days and nights to deliver a Doctor Baby.

until today, i NEVER take history of a patient..will do later ;)

1st & 2nd day: selamat pagi doktor :)

assalamualaikum readers :)


hehe.now i'm in second week for my training (nak masuk minggu ketiga da pun..weweweee)
ok.want to type about emm...yeah, my first day there..of course..as i told u before..(now I can access the internet even I not pay yet..hehe :)


1st day only a little to study. not so bertimbun. I introduced myself to the staffness
ok.let me tell u, :)


i was so nervous but I make myself to prepare any obstacles that i must encounter
just said to yourself 'I CAN'..wihout any doubt, start your move..
then, everything which impossible at the first thought will disappear..insyaallah


my elective posting a.k.a attachment a.k.a training in the first and second week: Surgery department. I will study a lot of cases here like gout, diabetes, hernia, neoplasm and gynaecomastia.


actually, I not so 'attach' to the doctor. Just Me. In other word...em I like to use suka hati..hehe

suka hati nak round wad mana ( wad 6B & 6C adalah wad pembedahan)
suka hati nak belajar apa...
suka hati nak tengok kes apa....blablabla..tambah suka hati kat depan, eh but,must OBEY rules..hihi


so, first day, in the morning, I followed a group of doctors..about 6-8 docs..then, of course, i was the youngest. I asked the KJ@head nurse@sister (aku panggil puan..haha,tak lama kemudian, kan aku asyik dok puan2 je, sister tu pun cakap, ktcn panggil sister.hehe..manala aku tahu..ngengenge :)

-the doctors (house officer & medical officer) were discussing about the cases.
they checked for vital sign like pulse, respiration, & blood pressure. any symptoms present for last 24 hours..and also seperti biasalah,tanya khabar pesakit..dressing also..
during ward round, pharmacist also participated the doctors group. Discussion among them HO, MO and HOD.


okeyh..hehe..btw, I did not understand about the topics they discussed about..8am-10am kot,baru habis round wad..2 hours(myb lebih,tak ingat) on foot..wahhh2x..adeyh. memang kat sini aku rasa cam buang masaaa sgt.tapi ni kan first day,relax la dulu. enjoy..satu lagi,kita boleh belajar cara..ala simple je sebenarnya patient-doctor relationship & communication, deal patients..tapi tu la..menyedapkan hati and think positive :) -always-


2nd day


I went to UPM library. studied about surgery.(tebal weh buku)..some. nothing much. lagipun, takde white coat/baju putih belum beli lagi. fikir nak beli ke tak nak ke nak ke tak nak..ish, lupa nak buat balik haritu..em


everyday, at home, I will revise all I got in hospital. in such a small matter, to the extravaganza matter..(betulke extravaganza?..hehe..ape2la~
